What happened

Lithonia Lighting has recalled certain indoor HID light fixtures used in commercial and industrial settings such as retail stores, warehouses, and gymnasiums. A component in these fixtures can leak fluid that degrades the acrylic lenses and reflectors, causing them to crack and fall. Falling acrylic pieces pose an injury risk to people below the fixtures.

The recalled fixtures have acrylic lenses and/or reflectors and were manufactured in Crawfordsville, Indiana between November 2002 and October 2003. Only specific models at certain wattages are affected, which can be identified on a label attached to the ballast housing. The recall also includes certain ballast repair kits. According to the record, building owners and managers should contact Lithonia to verify whether their fixtures are included in the recall and to arrange for a replacement fixture or faulty component. Lithonia and its distributors are directly notifying customers who purchased the recalled fixtures.

Hazard
