What happened

Roche Diagnostics Operations, Inc. is recalling ACCU-CHEK Tender I and ACCU-CHEK Tender II infusion sets (catalog number 04541421001, lots 5040427, 5044352, and 5061933) due to a potential defect where the tubing can become detached at the connect/disconnect location. These are devices intended for infusing or injecting fluids under the skin. A total of 13,041 boxes were distributed nationwide in the US.

If the tubing detaches during use, insulin delivery will stop without the pump alarming to alert the user to the interruption. The recall notice does not specify a remedy for affected consumers.
