What happened

Roche Diagnostics Operations, Inc. is recalling 82 cobas b 123 Fluid Pack COOX units (catalog number 05170036001) distributed nationwide in Louisiana, South Carolina, California, Texas, Nebraska, Virginia, and West Virginia, as well as Puerto Rico. The recall involves lot numbers 21446113 (expiration 9/5/2015) and 21446153 (expiration 9/24/2015). The cobas b 123 POC system is a fully automated device used to measure pH and other blood parameters in whole blood samples.

The recall was issued because a calibration issue with the PO2 parameter can cause the device to produce erroneously low PO2 results, particularly in blood samples with PO2 values below 50 mmHg. Quality control failures affecting primarily Levels 1 and 2 may not be detected because results can fall below mean values while still remaining within acceptable statistical limits.
