Scuba regulators and adaptors
Check against the official notice below — it lists the full affected range.
Aqua Lung is recalling certain scuba regulators and adaptors due to a drowning hazard. The problem occurs when a technician over-tightens the DIN retainer during installation, which can cause the brass retainer to break under pressure. When this happens, air rapidly escapes from the scuba cylinder and the regulator detaches from the cylinder, creating a life-threatening situation underwater.
The recall affects Aqua Lung Titan DIN 1st Stage regulators with serial numbers lower than 6062501 stamped on the regulator body, and Titan/Conshelf DIN adaptors marked "300 BAR MAX" on the side (adaptors marked "300 BAR MAXI" are not affected). The affected regulators contain brass DIN retainers manufactured before June 2006. The importer is voluntarily withdrawing the products from the market and recalling them from consumers, though the source does not specify what remedy is available to affected consumers.

The product poses a risk of drowning because over-tightening of the DIN retainer by a technician during installation can result in the retainer breaking under pressure, a rapid escape of air from the scuba cylinder and the regulator detaching from the scuba cylinder.
