A stepping Sound Mat designed for children over 12 months old is being voluntarily recalled due to a safety hazard. The mat is made of plastic foil featuring six animal figures and includes an attached "sound board" with a push button that produces animal sounds and blinking lights when pressure is applied to black and white spots on the animal images. Testing revealed that the individual layers of the mat slide on each other, causing the upper foil to twist, which creates an undesired slip hazard.

On the basis of a functionality test (in shoes, socks and bare feet), the individual layers of the mat slide on each other, causing the upper foil to twist, which represents an undesired slip. Considering the character and design of the product and the age limit for its use (>12 months), the risk of injury as a result of a slip and subsequent fall can not be excluded while using the product.
