What happened

Graco is offering a free retrofit kit for certain Carrier Swings and Convert-a-Cradle Swings manufactured between November 1991 and October 1992. When the carrier seat is in the fully reclined position while attached to the swing, infants can push themselves up and over the back of the seat, creating a fall hazard. The company reports that five infants fell backward out of swings and one infant was found unconscious suspended upside down, though no permanent injuries resulted. Approximately 208,000 swings were produced and distributed nationwide.

Affected consumers should call 1-800-345-4109 for a free retrofit kit and installation instructions, or write to Graco Children's Products, Inc., P.O. Box 100, Elverson, PA 19520. Consumers should first check if their swing's carrier seat handle adjusts to three positions—only those with three-position handles require the retrofit. Until the retrofit is installed, consumers should not use the fully reclined position while the carrier seat is attached to the swing.
